What is Personal Assistance Services and Supports (PASS)?
PASS is a CEDARR direct service that can help families of children with special health care needs. PASS is provided in the home or community setting to help improve the following:
· A child’s ability to do activities of daily life;
· A child’s ability to make self-preserving decisions;
· A child’s ability to participate in social situations.
Who’s Eligible?
Ø Children who are Medicaid eligible
Ø Children birth to 21 years old
Ø Children living with their family or legal guardian
Ø Children with chronic and moderate to severe cognitive, physical, developmental and/or psychiatric conditions
Ø Children who are already enrolled in a CEDARR Family Center and have a Family Care Plan
Family Role and Responsibilities
Ø Families recruit, train, manage and supervise their PASS worker
Ø Families manage identified service hours
Ø Families write the PASS Care Plan (with assistance from the PASS Agency as needed)
Ø Families collaborate and communicate with PASS agency staff
PASS Worker Role and Responsibilities
Ø Implements Care Plan
o Provides guidance and redirection
o Provides opportunities for social interactions
o Provides hands-on assistance
o Keeps progress notes on service plan goals
o Provides information to families on service plan goals
Ø Attends on-going training sponsored by PASS Agency
PASS Coordinator Role and Responsibilities
Ø One-hour direct contact per week to family for Service Plan implementation
Ø Activities include:
o Provide or arrange training for families in all aspects of program implementation
o Train or support families in recruiting, interviewing and orienting PASS Workers as well as back-up plan implementation
o Maintain contact with CEDARR Family Center
o Provide on-going assistance to help families and PASS Workers to maintain positive relationships
o Monitor monthly Program satisfaction with families
o Provide or arrange training PASS Workers on employee responsibilities
o Collaborate and support PASS families and PASS Workers
Clinical Consultant Role and Responsibilities
Ø Provides expert guidance to the child, family and PASS worker
Ø Observe child’s progress
o Provides recommendations for techniques and strategies to be used
Ø Review and approve service plans
Ø Provide guidance to the family to assist in managing and supervising the PASS worker
Ø Clinical consultation takes place in the home, at the PASS Agency or over the phone
Key Components of PASS
Ø Benefits of a Consumer-Directed Program like PASS
o PASS provides family greater choice in how services are delivered
o Helps decrease family reliance on traditional service delivery systems
o Builds on natural strengths and expertise of the family
o The family defines what should be done
o The PASS Agency describes how it could be done
o The family reviews the child’s progress with the Clinical Consultant and/or the PASS Coordinator
o The family reviews the PASS worker’s performance with the Clinical Consultant and/or the PASS Coordinator
o The family along with the Clinical Consultant and PASS Agency staff address renewal, transition planning, and/or discharge needs
Ø The family can designate someone to assist them in managing the program
Ø Does not take the place of services the child may be eligible for through Special Education, Early Intervention or Head Start
Ø May be provided along with other services (such as HBTS) as part of coordinated plan of care
Ø Has a maximum plan duration of 6 months, with unlimited renewals if eligible
Ø Transportation may be provided by PASS worker if identified in the PASS Care Plan and provided per PASS Certification Standards requirements
Reasons for Ending PASS Services
Ø The child loses Medicaid eligibility
Ø The child no longer requires 1:1 assistance beyond the typical age-level support
Ø The family chooses to discontinue PASS
Ø The family is not capable or willing to perform the tasks required to manage a consumer-directed program nor can they designate someone to assist them
Ø The family is no longer accessing services from a CEDARR Family Center
Ø The child is not benefiting from participation in PASS
Ø The level of service is no longer appropriate
Ø The child is at risk of harm to self or others
Ø The home environment presents safety risks to staff
Ø The parent/guardian is not successfully following program rules
Ø The child has been admitted to an institutional setting
